Description

We are seeking a compassionate and dedicated home care nurse to provide personalized medical care to patients in their homes. The role involves administering treatment, monitoring patient health, and offering education and support to patients and families. This position is essential for promoting independence and improving the quality of life of individuals receiving care outside of a clinical setting.

Provide direct nursing care to patients in their homes as per physician instructions

Administer medications, injections, IVs, and wound care

Monitor and document vital signs, symptoms, and progress

Perform assessments and update care plans regularly

Educate patients and family members on health conditions, medication management, and preventive care

Coordinate with physicians, therapists, and other healthcare professionals as needed

Maintain accurate, timely medical records and nursing reports

Ensure patient safety and adherence to health protocols
